As you might expect, there’s paperwork to do. You’ll save time on your first visit if you take a few minutes now to gather the items you’ll need for that first appointment. For your convenience, you can even download and complete the necessary forms and bring them with you to our office. If you are unable to fill out these forms before your first visit, please plan to arrive 30 minutes before your scheduled appointment.

Also, please bring your insurance card, a list of all current medications, copies of pertinent diagnostic tests and medical records, and referral forms, if necessary. Contemporary Women’s Health accepts most commercial insurance plans.
